Als je dit formulier invult nadat je de opleiding hebt gevolgd, gelieve hier dan een attest of bewijs van deelname toe te voegen samen met de factuur. Als je dit formulier op voorhand indient, vergeet deze documenten dan niet naar ons te mailen, ten laatste twee maanden na afloop van de opleiding.

If you fill in this form after you have completed the training, please upload a certificate of participation and the invoice. If you submit this form in advance, do not forget to mail these documents to us, at the latest two months after the end of the training.

Si vous remplissez ce formulaire après avoir suivi la formation, veuillez ajouter une attestation de participation et la facture. Si vous soumettez ce formulaire à l'avance, n'oubliez pas de nous envoyer ces documents par courrier, au plus tard deux mois après la fin de la formation.
